Adverse incident review

Adverse incidents should be recorded through internal trust risk processes and the SONeT clinical transport lead should be notified. Please email the respective SONeT hub when an adverse incident form has been completed. This process is for feedback by professionals only.

Parents should use the SONeT parent information feedback form which can be found on our feedback for parents page . Parents and families may wish to contact the patient support service at their local hospital.
